Surgical menopause: after oophorectomy, what happens next

Bilateral oophorectomy — removing both ovaries — causes an instant, complete drop in estrogen and testosterone. There is no perimenopause runway. Symptoms hit within days, and long-term consequences without HRT are substantial. For most women under 45, HRT is not optional.

The long-term stakes without HRT

  • Cardiovascular disease risk up 80–120% if under 45
  • Dementia risk elevated, especially if under 45
  • Osteoporosis accelerated
  • All-cause mortality elevated
  • Sexual dysfunction, mood disorders, cognitive decline

Treatment framework

  1. Start HRT within days of surgery — do not wait for symptoms
  2. Transdermal estradiol at replacement dose (0.075–0.1 mg patch)
  3. No progesterone needed if uterus removed (unless endometriosis history)
  4. Consider testosterone if libido/energy remain low
  5. Continue at least until age 51; often longer

Frequently asked questions

How is surgical menopause different?

Sudden and severe. Estrogen goes from premenopausal levels to near-zero overnight, along with 50% of testosterone production. Symptoms are more intense than natural menopause.

Do I need HRT after oophorectomy?

Under 45: yes, unless contraindicated. 45–50: strongly recommended. 50+: individualized based on symptoms and risk.

What about testosterone after oophorectomy?

Ovaries produce ~50% of a woman's testosterone. Consider adding testosterone if libido, energy, or motivation remain flat on estrogen alone.

Was my ovariectomy necessary?

For hormone-positive cancer, high-penetrance genetic risk (BRCA), or severe endometriosis: yes. For benign disease, ovarian conservation is increasingly favored.

What if I had a hysterectomy but kept my ovaries?

You went through menopause at a normal age but without periods to mark it. Symptoms and treatment are the same as natural menopause.
