Rapamycin for longevity: what women in midlife should know

Rapamycin (sirolimus) inhibits mTORC1, a master regulator of cell growth and aging. It is the single molecule with the most consistent lifespan extension across yeast, worms, flies, and mice — including when started in midlife. Mechanism: hitting mTORC1 without mTORC2

mTORC1 senses nutrients and drives protein synthesis and cell growth. Chronic mTORC1 activation accelerates aging, promotes senescence, and impairs autophagy. Weekly rapamycin transiently inhibits mTORC1 without significantly hitting mTORC2 (which handles insulin signaling) — this is why intermittent dosing has a favorable metabolic profile.

  • Elderly humans given a rapamycin analog (RAD001) had improved vaccine response and reduced infection rates (Mannick et al., Sci Transl Med 2018).
  • The PEARL trial (2024) showed improved lean body mass and pain in women taking 5–10 mg/week for 48 weeks.

Rapamycin has been proposed to slow ovarian aging in reproductive-age women — small trials are underway. In perimenopausal and menopausal women, current use focuses on healthspan (immune function, body composition, cognitive resilience).

Does rapamycin extend lifespan in humans?
