Why does cholesterol worsen in menopause?

Estrogen upregulates LDL receptors in the liver. When estrogen drops, LDL clearance slows and LDL rises. HDL also drops. Triglycerides increase.

Does HRT improve cholesterol?

Yes. Oral estrogens raise HDL and lower LDL dramatically, though they also raise triglycerides. Transdermal is neutral-to-favorable overall with better safety.

HRT or statin for cholesterol?

Not either/or. If ASCVD risk is high, statin. If in the HRT window and symptomatic, both. HRT is not a substitute for statins in high-risk patients.

What is the "window of opportunity"?

Starting HRT within 10 years of menopause (or before age 60) confers cardiovascular benefit. Starting later may not — timing matters.

What about ApoB and Lp(a)?

ApoB is a better cardiovascular predictor than LDL. Lp(a) is genetic and doesn't change with lifestyle. Both should be measured once in midlife.
