When to start HRT: the timing window that matters

The "timing hypothesis" suggests starting HRT closer to menopause has a more favorable risk-benefit profile.

NAMS guidance is clear: for healthy women under 60 or within 10 years of menopause onset, the benefits of HRT generally outweigh the risks. Outside that window the calculus is more individual.

Why earlier is better for most patients

Estrogen receptors and vascular tissue respond differently to hormone therapy depending on how long they have been without estrogen. Starting earlier appears to support cardiovascular and bone outcomes more reliably.

There is no benefit to suffering through symptoms while you "wait it out." If you are bothered, talk to a clinician now.
