HRT and migraines: continuous transdermal wins
Migraines — even with aura — are not an absolute contraindication to HRT. What matters is delivery. Cyclical oral estrogen creates the estrogen fluctuations that trigger migraine. Continuous transdermal estradiol at a steady low dose usually improves headaches.
Migraine-safe HRT protocol
- Continuous transdermal estradiol
- Continuous progesterone (no cyclical withdrawal)
- Start at lowest dose, titrate slowly
- Track headache frequency for 8–12 weeks
Frequently asked questions
Can I take HRT if I have migraines with aura?
Yes, with continuous transdermal estradiol at the lowest effective dose. Oral cyclical regimens are avoided.
What about stroke risk?
Transdermal estradiol does not raise stroke risk in the way that combined oral contraceptives do.
Which route is best?
Patch or gel, continuous, no cyclical breaks.
