It is intended for women with an intact uterus who need both estrogen (for symptom control) and progesterone or a progestin (for endometrial protection). The patch is changed once or twice weekly depending on the formulation.
Two combination transdermal products are widely available: estradiol/norethindrone acetate (Combipatch) and estradiol/levonorgestrel (Climara Pro). Both are FDA-approved for menopausal symptom management in women with an intact uterus.
The estradiol component restores physiologic estrogen signaling in the brain, bone, cardiovascular system, and urogenital tissue — reducing hot flashes, improving sleep, and preserving bone density. The progestogen component prevents estrogen-driven endometrial proliferation, protecting against endometrial hyperplasia and cancer. Transdermal delivery of both components avoids hepatic first-pass metabolism.
Randomized trials of combination transdermal patches (Utian et al., 2005; Speroff et al., 2003) demonstrate significant reduction in vasomotor symptoms, effective endometrial protection, and generally favorable tolerability compared with oral combination products. The NAMS 2022 Position Statement includes combination transdermal HRT among recommended options for symptomatic menopausal women with an intact uterus.
Because progestogens in combination patches are synthetic (norethindrone acetate or levonorgestrel) rather than micronized bioidentical progesterone, individual tolerance and breast tissue effects can differ from bioidentical protocols and are considered in prescribing decisions.
