The estradiol patch: how to use it, what to expect

The estradiol patch is the workhorse of modern HRT: bioidentical, transdermal (bypassing the liver), and dose-flexible. It comes in twice-weekly and once-weekly formulations, in doses from 0.025 mg to 0.1 mg. Here's how to use it, common pitfalls, and how doses are adjusted.

Typical dosing

  • Starting: 0.05 mg patch
  • Low end for younger women or those sensitive: 0.025 mg
  • Higher end for POI, surgical menopause, or persistent symptoms: 0.075–0.1 mg

Application tips

  1. Clean, dry, hair-free skin
  2. Rotate site each application (avoid same spot for 7 days)
  3. Press firmly for 10 seconds
  4. Do not cut patches — this alters delivery
  5. Never apply to breast tissue

Common pitfalls

  • Applying to lotion-treated skin — the patch won't stick
  • Same site twice in a row — skin irritation
  • Missing a change day — symptoms return within 24–48 hours
  • Cutting the patch — dose becomes unpredictable

Frequently asked questions

Where do I apply the patch?

Lower abdomen, hip, buttock, or thigh. Rotate sites each application. Never on the breast. Clean, dry, oil-free skin.

How often do I change it?

Depends on the brand. Vivelle-Dot and Alora: twice weekly. Climara: once weekly.

Does it stay on in the shower?

Yes. Water is fine. Avoid oils, lotions, and heavy sweating on the site.

What if it falls off?

Reapply if still sticky. If not, apply a new patch on a different spot and continue the original schedule.

How do I know if the dose is right?

Symptom control is the primary endpoint. If hot flashes, sleep, and mood improve, the dose is working. If not, titrate up.
